Kim Cuddy, Topaz Christian and Tiffany Bellamy Receptionists
Will answer the phone when you call in to 276-623-8100. During busy times you may have to wait for your call to be answered but you will not be switched to voice mail at the front desk, as soon as a receptionist is free and they will pick up your held call. If you would like to email long-term appointments request or cancellations you may at recmedpedsa@eva.org. We try very hard to reduce the amount of time you'll have to wait to see out the physicians and nurse practitioner, however we also try to see all sick patients on the day you call in, so please understand we may not be able to give you an appointment at exactly the time you request and appreciate your cooperation as we try to reduce the overall wait for everybody.
Cindy Blevins and Michael Sneed Medical Records
Will help you if you need a copy of your medical records or other related medical records requests. Please note we are unable to fill out school required physical forms unless we have seen your child recently. We would be happy to schedule a physical to allow you to get the required paperwork completed. We do not charge for medical records, if we are sending you to a new doctor for example, a specialist. However, if you simply need a copy or you're transferring to a new office we will charge you a copying fee. Under Virginia statute the original medical records remains the property of the practices however, you always welcome to get a copy of them. If you need us to send your records to another office, please use the medical transfer form. HYPERLINK TO FORM You may use the same form to request that records are sent to us.
